What file format does the Questionnaire Auto-Filler expect?

An Excel file with up to three columns: Category (optional, adds context to the question), Question (required, one unique question per row) and Context (optional additional information about the question). A sample Excel file can be downloaded from the upload page under "Excel File Formatting Instructions".
